USA: National Quality Forum Expands “Reportable Incidents”

USA: The National Quality Forum this week expanded the list of preventable medical harm scenarios that cause death or serious injury to patients. They propose adding 4 more situations, which would bring the list to 29. Twenty-four states are required (at this time) to use the same reporting method. (Why not all?) This list has been in place since 2006 but according to this story, it hasn’t helped much. Hospital accountability and public reporting remains abysmal. Kevin B.
